Maryland License Plates

Request the MD-VR 124 Form for the Maryland organizational license plates for Alpha Phi Alpha Fraternity.

Applications requests will be forwarded to the organizational representative within Delta Lambda Chapter, Bro. R. A. Mills (443) 228-8726, who will email you an “Application/Certificate for Organizational License Plates” from the Maryland DMV.

Once you receive the application, fill in the appropriate sections and mail your application with your $25 fee to:

Eligible Vehicles are passenger cars, multi-purpose vehicles, motorcycles, and trucks (10,000 lbs. or less)

Please note MVA charges a fee of $25 to cover the cost of the specialty plate. Once processed by MVA, the tags will be mailed (or given to you if in-person) and you’ll be able to proudly display your new Fraternity tags on your vehicle.

You can obtain your organizational tags in person at your local Maryland DMV. Present the completed “Application/Certification for Organizational License Plate,” and fee, along with your existing tags to receive your new organizational plates. Please be aware that not all locations carry all specialty tags and it is advised you call the DMV branch before you visit. Please look up your DMV office location here.

PLEASE ALLOW AT LEAST 48 HOURS to prepare and return the MD-VR 124 form.
